The anamnesis certificate is a medical-legal document required in many situations regulated by Italian law. It certifies that a citizen does not present any physical or psychological conditions that would make them unfit for certain activities, such as driving, firearm possession, or participation in public competitions.
It is issued by an authorized physician, typically the general practitioner, but it can also be issued remotely by professionals qualified to use a digital signature, in accordance with Presidential Decree no. 445/2000 and the Digital Administration Code (Legislative Decree no. 82/2005).
What Is the Anamnesis Certificate
The anamnesis certificate is an official declaration of the patient’s medical history. It evaluates the presence or absence of potentially disabling conditions and is required by law for roles involving public safety or legal responsibility.
The certificate takes into account:

Cardiovascular, neurological, endocrine, or psychiatric disorders

Visual and hearing impairments

Use of medications that affect cognitive or reactive abilities

Dependence on alcohol or psychoactive substances
-
Previous hospitalizations, surgeries, or relevant diagnoses
The physician is legally required to objectively assess the patient's clinical condition and report any potential risks.
When It Is Mandatory
The anamnesis certificate is legally required in the following cases:

Issuance or renewal of a driver’s license (Art. 119, Italian Highway Code)

Request or renewal of a firearm license (Law 110/1975, Art. 35)

Participation in public competitions requiring medical fitness (e.g. Armed Forces, Police, Firefighters)

Application for a nautical license (Ministerial Decree 146/2008)

Employment in positions involving high responsibility or risk

Participation in competitive sports, if required by a sports physician
Who Can Issue It and How It Works
The certificate can be issued by:

A general practitioner (GP)

A physician from the National Health Service (SSN)

A licensed private physician authorized to issue medical certifications
Issuance methods:

In person, at the doctor’s office

Online, if the physician is authorized to use a digital signature (Art. 20 of the Digital Administration Code)
In both cases, the certificate holds full legal validity, provided that it is:

Signed by the doctor (either manually or digitally)

Issued on official letterhead or using an approved form

Released within three months of the date of submission to the requesting authority
Patient Obligations and Legal Responsibility

The patient is required to provide accurate and complete information.

The physician may only certify information they can verify and confirm.

False declarations may constitute the criminal offense of making false statements in public documents (Art. 483 of the Italian Penal Code).

The certificate may be checked by authorities at any time.

In the case of incompatible medical conditions, the physician has the right and duty to deny the issuance.
